How it works

Build the form, put it on your website and follow every answer through to a contact, a lead and a reply, all in one place.

  1. Step 1: Start from a template, or describe it

    Pick one of 100+ ready-made forms for your industry, or tell the AI what you need in a sentence. You get a complete draft with fields, steps and rules, and every part of it stays editable.

  2. Step 2: Drag fields into columns and steps

    Over 40 field types, from name and address to file uploads, signatures, ratings and matrices. Put fields side by side, split long forms into steps with a progress bar, and preview every change on desktop and mobile.

  3. Step 3: Add logic and calculations

    Show, hide or require fields based on earlier answers, and skip steps that do not apply. Calculations work out estimates, totals and scores while people type.

    Logic rules: show Pet details when Any pets is Yes, skip to Contact when Service is Standard, and a price calculation of bedrooms times 30 plus bathrooms times 20 plus extras.   4. Step 4: Make it look like your website

    Start from a theme, then set your colors, font, corners and spacing. Forms size themselves to your layout and work on every screen.

  5. Step 5: Publish it wherever people are

    Add it with the WordPress block or shortcode, paste one script tag on any other website, or share a link and QR code. It can also open as a popup after a delay, on scroll or when someone is about to leave.

  6. Step 6: Every submission in one inbox

    Read, star, assign, tag and add notes to submissions from every form on every website. Spam goes to its own folder, and you can export to CSV whenever you need to.

  7. Step 7: Contacts, leads, emails and webhooks

    Each submission creates or updates a contact, the same contacts Live Chat uses, and can open a lead in your pipeline. Your team gets an email, the visitor gets a confirmation, and your other tools can get a webhook.

    Follow views, starts, completion rate and submissions for every form, and see where people drop off. Survey questions get their own reports, from rating averages to your Net Promoter Score.

  1. Install and activate the PlugMatrix plugin, then click Connect to link your account.
  2. Open Forms in your WordPress admin and pick a form, or start one from a template.
  3. Add it to any page with the Form block, or paste its shortcode: [plugmatrix_form id="FORM_ID"]

Add the script once, then place a form anywhere with one line. Replace FORM_ID with the id from the form's Share tab.

How do you stop spam?

Every form has a hidden honeypot field, a minimum time to fill it in and a limit on how many submissions one network can send per hour. You can also block keywords and throwaway email addresses, and turn on Cloudflare Turnstile, hCaptcha or reCAPTCHA. Spam goes to its own folder and never creates contacts, leads, emails or webhooks.

What about privacy and GDPR?

You decide what each form asks for, add a consent checkbox that links to your privacy notice, and choose how long submissions are kept. Submissions are linked to contacts, so when someone asks, you can export or erase what you hold about them. PlugMatrix does not fingerprint visitors, and you stay in control of your own privacy notice.

Where are uploaded files stored?

Files and signatures are kept in private storage, not in your WordPress media library or on your web server. Your team opens them from the dashboard through short-lived links. Each upload field can limit file types and sizes (up to 25 MB per file), executable files are never accepted, and stored files count toward your plan.

Does it work with Live Chat?

Yes. Both products use the same contacts, leads and team. Someone who chats with you and later fills in a form is one contact, with the conversation and the submission in the same history.

Can I change the templates?

Yes. There are 100+ templates across industries, from contact and quote requests to event registrations, job applications and customer surveys. Each one becomes a normal form in your account: change any field, rule, email or style.

Can I make conversational forms?

Yes. Switch a form to conversational mode to show one question at a time, like a chat. It uses the same fields, logic and inbox as the classic layout, so you can try both.

Does it work on WordPress?

Yes. The PlugMatrix WordPress plugin adds a Forms menu to your WordPress admin, a block for the block editor and a shortcode for everything else. The same plugin also runs Live Chat.
